New patients are required to participate in a new patient examination
on registering with the practice. In addition new patients will be
asked to complete a simple questionnaire that will be used to provide
us with medical history pending receipt of full medical records from a
previous doctor. An appointment will be made for new patients to see
the practice nurse at mutually convenient time.
